Check For Duplicate Insulins on Discharge Orders

There'll be more confusion about concentrated insulins...Humalog U-200, Toujeo U-300, Tresiba U-200, and Humulin R U-500.

Most concentrated insulins are more or less "me-toos" of the nonconcentrated forms...and there's no proof the newer concentrated analogs lead to better glycemic control. So don't expect them to be on your hospital's formulary.

But it's important to keep them straight during care transitions.
