Explain to prescribers that loading doses are needed for concentration-dependent meds, such as levofloxacin. It takes longer to reach therapeutic blood levels when doses are spread far apart.

If levofloxacin is a new start, generally give 750 mg for one dose, then 500 mg every OTHER day...instead of 750 mg daily. But skip the loading dose if switching from IV to PO levofloxacin.
