Assess Med Options When Standard Heart Failure Therapy Isn't Enough

We're getting questions about managing patients admitted with heart failure exacerbations despite standard therapy.

First verify adherence to target doses of meds that decrease mortality in heart failure with reduced ejection fraction (HFrEF)...an ACEI or ARB, an "evidence-based" beta-blocker (carvedilol, etc), plus an aldosterone antagonist. About half of patients may not be adherent.
